Our mission to serve patients with special financial circumstances ensures that no one requiring medical services is turned away, regardless of their current insurance or employment status. In order to receive low and no cost services, we qualify each patient for the programs for which he/she is eligible. This process involves filling out paperwork and takes time. For your appointment, we ask that you bring the following items:

1. Photo Identification

2. Basic patient information

3. Insurance Card (if any)

4. Proof of Income (if no insurance)

5. Immunization Record (if pediatric)

6. List of current medications (if any)

Please come to your scheduled appointment 15 minutes early. We will do all we can to get you in on time, but please expect that there may be delays. We appreciate your patience.
